Read and sign the RIDE-ALONG APPLICANT ACKNOWLEDGEMENT of HIPAA Obligations Details: The Requester agrees to maintain the confidentiality of patient health information and respect patient privacy.

Once complete, please email all documents to fireweb@cityofdavis.org.

Once you have read the Overview & Guidelines and submitted the FORP packet you will be contacted by a representative of the Fire Department to schedule/confirm your Ride-Along if your packet is approved.
