Claim Against the City of Davis
A claim must be filed with the City of Davis within a period provided by state statute after which the incident or event occurred. Be sure your claim is against the City of Davis and not another public entity. Both sides of the Claim Form must be completed in full, signed and date. It is recommended to include copies of invoices, photographs and any other supporting documents with the claim form. No documents will be returned. The signed and completed claims must be mailed or delivered to:
