Please keep in mind that this form cannot be filled out by residential tenants. Utility bills must still be sent to the property owner, unless the property will be in the care of a property management company.

A residence that is to be vacant for a minimum of 30 days or more is entitled to the $35.65 per month No Sanitation Service rate. The notification must be given to the city prior to the vacancy and the following affidavit must be signed and returned to the Finance Department.
