- Pick up a medical authorization form or download one. This form covers attendance to dances and the drop in programs. This form is good for the school year! A new one will need to be completed at the end of the school year or before school starts in August!
- Field Trips and lock-ins have permission slips good for that event or trip only! These are available at Third and B.
